Check List: One-On-One Mentoring

Before the Match

▢ Review Keys to Success with your Mentor and the Student/Parent/Guardian Handbook

▢ Fill out and Return Student Commitment Form (needs to be updated every school year)

▢ Fill out and Return Information Exchange Sheet (needs to be updated every school year)

▢ Complete Preparing for the First Meeting

▢ Return Permission Form to Mrs. Sternweis

Day of 1st Match Meeting – Meet along with Mrs. Sternweis

▢ Pick up your pass in the Main Office to excuse you from class

▢ Take Preparing for the First Meeting along to your mentor meeting

▢ Arrive on time; conduct yourself in a professional manner

▢ Introduce yourself & thank your mentor for sharing their time

▢ Set up the next meeting date & time with your mentor & put it on your calendar

Day of each subsequent Match Meeting

▢ Meet with your mentor 1 hour per month during the school year

▢ Dress appropriately

▢ Pick up your pass in the Main Office to excuse you from class

▢ Bring any questions you have — here are some suggestions

▢ Arrive on time; conduct yourself in a professional manner

▢ Thank your mentor for sharing their time

▢ Set up the next meeting date & time with your mentor & put it on your calendar

▢ Fill out the Activity Log (needs to be completed for every meeting)

▢ Check-in with Mrs. Sternweis monthly
